The Secretary of State for Health and Care and the Chief Executive of NHS England have announced a rapid review of how care is co-ordinated between different parts of the health service. This includes how referrals are made, how patients are discharged from hospital and how the different parts of the health service communicate with each other and with patients, safely and effectively.
We are part of an engagement group meeting with policy teams seeking to hear views on what’s working well, what is working less well, and what you think could be done differently. Please share any insight, issues and case studies via [email protected] by 15 November.
